The Order of Malta Lebanon has been operating in the health and social welfare sectors since 1957. It manages a network of 60 projects serving the most vulnerable populations, across the Lebanese territory including primary healthcare centers, mobile medical units, elderly day care centers, hosting centers for the differently abled and projects for the displaced and refugees as well as an agro-humanitarian project within the catchment areas of its centers.
The Order of Malta Lebanon has developed a comprehensive multi-annual program to strengthen its social, agricultural and health services as well as its structures.

SUMMARY
Social Worker supports vulnerable individuals, families, and communities in addressing social, economic, and psychosocial needs, with the objective of improving well-being, protection, and access to essential services.
The role involves conducting comprehensive social assessments, planning and implementing appropriate social interventions, and ensuring follow-up through services delivered at the Primary Healthcare Center and during outreach activities, including medical days, home visits, and community-based actions.
RESPONSIBILITIES
· Conducts Family social assessments at the Primary Healthcare Center and during outreach activities, including home visits and medical days.
· Creates, updates, and maintains beneficiaries’ files and databases, ensuring accuracy, completeness, and confidentiality.
· Actively participates in outreach activities, including medical days, home visits, and field missions, to identify needs, provide social support, and ensure access to services for vulnerable populations in remote or underserved areas.
· Assesses community-level needs during outreach activities and contributes to adapting interventions to local priorities and vulnerabilities.
· Plans and delivers awareness and information sessions within the center, schools, and other community settings, focusing on social issues, protection, access to services, and prevention of social and health-related risks.
· Engages with community leaders, municipalities, local organizations, and other stakeholders to strengthen community participation, referrals, and social support mechanisms
· Participates in the organization, prioritization, and implementation of distribution activities, ensuring fairness, dignity, and accountability
· Receives distribution supplies when required and supports their allocation according to identified priorities and project guidelines.
· Provides information, guidance, and orientation to beneficiaries seeking social, legal, or protection-related support.
· Refers cases requiring specialized follow-up to relevant internal and external services, including psychologists, psychiatrists, legal aid providers, and protection actors.
· Coordinates closely with medical staff to support patients facing social barriers to treatment adherence and access to care.
· Logs all activities, assessments, and interventions in PHENICS and relevant data collection tools, ensuring timely and accurate data entry.
· Prepares weekly and monthly reports on activities, outreach actions, distributions, and case follow-up, highlighting progress, challenges, and outcomes.
· Participates in monitoring, evaluation, and learning processes to improve the effectiveness and quality of social interventions
· Participates in trainings, meetings, and supervision sessions to maintain and enhance professional knowledge and skills.
· Applies and adheres to the code of ethics, mission, vision, and values of Order of Malta – Lebanon at all times.
· Performs any other related duties assigned by the Healthcare Center Administrator or Social Worker Coordinator in line with project needs.
QUALIFICATIONS
· Bachelor degree in social work (assistante sociale). Masters degree is a plus
· A minimum of 3 years relevant experience as a social worker including social needs assessment
· Working knowledge of social theories and practices
· Knowledge of mental health in complex situations (IASC guidelines, Psychological First Aid, mhGAP…) and human rights based approach in mental health
· Knowledge of legal and ethical issues related to patients' rights and social legislation in Lebanon
· Social interview and assessment techniques
· Communication and strong active listening skills

· Ability to work in a diverse, multi-cultural and multi-ethnic environment
· Ability to adhere to ethical considerations and principles with regard to work with vulnerable populations
· English is a must in addition to Arabic. Fluency in French is a plus
· Proficiency in computer skills (Word, Excel, PowerPoint…)
· Working experience with Phenics is a plus
