The Order of Malta Lebanon has been operating in the health and social welfare sectors since 1957. It manages a network of 60 projects serving the most vulnerable populations, across the Lebanese territory including primary healthcare centers, mobile medical units, elderly day care centers, hosting centers for the differently abled and projects for the displaced and refugees as well as an agro-humanitarian project within the catchment areas of its centers.
The Order of Malta Lebanon has developed a comprehensive multi-annual program to strengthen its social, agricultural and health services as well as its structures.

SUMMARY
Responsible for planning and supervising all outreach and community-based medical activities outside the center, ensuring effective implementation of the project.
The coordinator acts as the focal point for community engagement, supports and organizes screening and awareness campaigns, medical days and conducts assessment and mapping to align activities with local needs.
RESPONSIBILITIES
· Plan, coordinate, and supervise all outreach and community-based medical activities within the target areas.
· Conduct service mapping to identify local resources, gaps, and priority health needs.
· Develop a clear intervention schedule based on identified needs and in coordination with the OML Healthcare Center Administrator
· Plan and prepare for outreach medical days, home visits, including site selection, scheduling, mobilizing CHC human resources, medical, logistical, and materials based on field needs and in close collaboration with the Healthcare Center Administrator
· Support field operations during medical days, including patient flow, registration, triage, and referral documentation.
· Ensure that vulnerable patients are appropriately referred to CHC services, secondary and tertiary care facilities, and coordinate transportation to facilitate access to consultations, laboratory tests, imaging, and specialized treatment
· Report referral trends, challenges, and barriers to the Program Manager and Healthcare Center Administrator to inform planning and improve access pathways.
· Establish and strengthen community groups and ensure continued engagement and mobilization within intervention areas.
· Coordinate with other OML programs to ensure complementarity and shared resources when applicable.
· Support data collection, field documentation, and reporting for all outreach, referral, and medical day activities.
· Applies and follows the code of ethics, values, mission, and vision of the Order of Malta-Lebanon.
· Performs other related duties assigned by the Program Manager and the Healthcare Center Administrator.
QUALIFICATIONS
· Bachelor’s degree in public health, Nursing, Social Sciences, Health Management, or a related field, Master’s degree is a plus.
· 5 years of experience in a hospital or healthcare center
· Experience with referral pathways, patient follow-up, and coordination with healthcare facilities (CHC/PHCC, hospitals, labs) is strongly preferred.
· Ability to work in a diverse, multi-cultural and multi-ethnic environment
· Initiative and drive for results
· Excellent communication skills, collaboration, and teamwork
· Patient with excellent problem-solving skills
· Proficiency in Computer literacy (Word, Excel…)
· Fluency in Arabic, English and French (written and spoken)
· Working experience with PHENICS
